This is a 1:1, 2-month, completely individual health coaching program. This covers cost of any and all care needed, including lab work. This commitment grants the patient access to Dr. Nick directly. The patient will have permission to text/call 24/7.

Hardship Agreement

Here at OFH, we want everyone to be able to have access to functional healthcare.

That is why we have implemented a plan agreement that we hope will allow more people to receive care and find relief from a functional and very patient specific approach.

This agreement will be made at the discretion of Dr. Nick based on the needs of the patient.
